Warm Exchange Plus (WE+)
is a new model of healthcare in
your community

The WE+ model of care aims to improve people’s health, their healthcare experiences, and wellbeing outcomes, while also supporting them to address the impacts of social issues on their health.  

Eligible people (that is Māori and Pacific people and those living in Quintile 5 areas with two or more Long Term Health Conditions (LTCs), and their whānau living with LTCs, in South Auckland) will be able to enroll in the WE+ model by talking with any one of the five service providers. You will begin a 12-week navigation journey, during which your needs will be identified and a tailored wellbeing plan put in place, with input from you and /or your whānau.

Our WE+ Health Navigators will be there every step of the way

Navigators will assist you and whānau in any way that they can to access services, and to support your experiences if you need to go to hospital or outpatient clinics. Navigators will also refer you and whaanau to services in the wider community where they know and trust high quality, culturally appropriate providers who will care for you as they do. Navigators will be your advocates when you are not feeling heard or understood. Together with your navigator, you will work toward whānau-created, whānau-centered, and whānau-led care plans and goals, so that you feel supported to be self-managing.

Who are we?

WE+ is a collective of like-minded healthcare organisations who chose to work together to deliver the new model. The five service providers are:

– offers free community health and wellbeing programmes in Manukau and West Auckland

Greenstone Family Clinic

ZOOM Pharmacy
– an innovative provider of virtual/ remote medicine services across all of NZ, including home delivery
